Please note that this form is meant for renewal purposes only and cannot be used for submitting a request to become a judge. Kindly fill out the form and wait for a confirmation of your renewal. It is important to note that all judges must be current AMA members and qualified and approved by the AMA Judge Committee.
There are two options below: one is just a judge’s card, and the second includes both a voting membership and a judge’s card.
